To submit a donation or sponsorship request, please complete the form below and submit a current W-9 for your organization. Return BOTH of these forms to any of our office locations. Your request will not be considered without both forms.
Our donation and sponsorship committee meets monthly. Requests must be made a minimum of 6 weeks in advance of your event. We will contact you if your request has been approved.
